The types of personal information you may provide include individual and business contact information (such as name, physical address, email address, and telephone or fax number), prior addresses and length of time at each address, username and password to access the Careers Sites, citizenship and work authorization status, driver's license number, driving history and other details about government-issued identification documents, education history, employment and salary history, military service information, resume or C.V., age range and date of birth, language proficiency, criminal record information, information about your relationship with UPS, other information about your qualifications, and requests for disability accommodations.
